How a Care Plan Runs

Six stages, organised as a continuous loop. Each cycle of measurement updates the next cycle of care — so a care plan that's twelve months in is the product of twelve months of accumulated learning.

    A structured care plan is generated, reviewed by the patient's physician, and agreed with the family before service begins.

    The platform assigns caregivers based on clinical specialty, geography, and existing patient relationships — continuity by default.

    Care is executed at home — to the same protocols, with the same documentation, every visit.
